HF/PAT Program Assistant

LOCATION: Dover, New Jersey- Hybrid (Office/Remote) Work Environment

CLASSIFICATION: Full-Time/Non-Exempt- 37.5 hours/week

ANNUAL SALARY: $40-$45k *signing bonus*

PRIMARY FUNCTION:  Responsible for conducting quality assurance calls and program data into the management systems. Handle administrative and program tasks such as conducting pregnancy testing, conducting outreach, scheduling assessments, etc for the HF Morris and PAT Bergen, Morris, Warren Programs.

ESSENTIAL DUTIES:

  • Enter data in a timely and accurate manner into a web-based information system
  • Coordinate data collection and compile data for reports
  • Conduct Quality Assurance calls/surveys to participants
  • Conduct pregnancy testing for program recruitment including providing prenatal care referrals
  • Conduct outreach and entering outreach disposition as needed
  • Provide coverage for home visitation services when requested
  • Prepare home visiting documentation
  • Schedule assessments and home visit appointments
  • Conduct administrative tasks such as; edit, type, reproduce and distribute correspondence/reports
  • Take minutes at meetings—compose, type, and distribute minutes
  • Maintain an organized and confidential filing system
  • Coordinate participant workshops/events, as needed
  • Travel to events, client’s homes, and/or meetings
  • Attend Community Advisory Board (CAB) and Community Network Association (CNA) meetings
  • Handle other assignments as requested.

PHYSICAL DEMANDS:   Work is performed in a variety of office, health care and community settings; may involve bending, color identification, gripping with hands and fingers, hearing voice conversation, keyboarding, kneeling, lifting up to 35 pounds, pulling, pushing, reaching, sitting, standing, stooping and walking.  Successful performance requires specific vision abilities that include close vision.  Employee will attend meetings at a variety of places and will need to drive.  Occasional stair climbing is required.

PHYSICAL EXAMINATION:  Offer of employment is contingent upon final candidate’s successful completion of a physical examination.

WORK REQUIREMENTS: There are no unusual work conditions. Noise level at work is moderate. The employee will experience outside weather conditions while in transit to and from meetings or during special events. This position requires the employee to own a functional vehicle, maintain a valid driver’s license and updated vehicle registration and insurance at all times. Employee must also have stable internet access.

MINIMUM EDUCATIONAL QUALIFICATIONS:   Associates degree preferred. High School graduate required.

MINIMUM EXPERIENCE REQUIRED:  Two years data entry and administrative experience required.  Computer proficiency in MS Office. Excellent verbal and written communication skills. Bilingual English/Spanish.
